Patient Eligiblity | Elegibilidad del paciente

Community-Based Program Eligibility – Children
Children ages 4 to 17 or who are enrolled in a secondary school are eligible for an eye exam and glasses.
Community-Based Program Eligibility – Adults
Adults 18+ are eligible for an eye exam and glasses if one of the following can be met:
  • The patient is referred by CommUnityCare, People’s Community Clinic, Lone Star Circle of Care, Volunteer Health Clinic, Project Access, Health Alliance for Austin Musicians (HAAM), Vivent Health. (Patients enrolled in MAP or MAP Basic are assigned to one of these partners.)
  • The patient is enrolled in Medicaid (specific plans TBA).
  • The patient is uninsured AND has a primary care physician at a Federally Qualified Health Center or look-a-line, or at a Nonprofit Health Center such as Hope Clinic, Lirios, Integral Care, Planned Parenthood, Samaritan Health Ministries, etc.
Late Arrivals
Patients should plan to arrive 15 minutes before their scheduled appointment to complete paperwork and pre exam measurements. Patients that do not arrive early, will have to reschedule for a later appointment date and time.

Elegibilidad para programas comunitarios: niños
Los niños de 4 a 17 años o que estén matriculados en una escuela secundaria son elegibles para un examen de la vista y anteojos.

Elegibilidad para el programa comunitario: adultos
Los adultos mayores de 18 años son elegibles para un examen de la vista y anteojos si se cumple uno de los siguientes requisitos:

  • El paciente es remitido por CommUnityCare, People’s Community Clinic, Lone Star Circle of Care, Volunteer Health Clinic, Project Access,
  • Health Alliance for Austin Musicians (HAAM) y Vivent Health. (Los pacientes inscritos en MAP o MAP Basic se asignan a uno de estos socios).
  • El paciente está inscrito en Medicaid (planes específicos por confirmar).
  • El paciente no tiene seguro Y tiene un médico de atención primaria en un centro de salud calificado a nivel federal o en un centro de salud sin fines de lucro como Hope Clinic, Lirios, Integral Care, Planned Parenthood, Samaritan Health Ministries, etc.

llegadas tardías
Los pacientes deben planear llegar 15 minutos antes de su cita programada para completar la documentación y las mediciones previas al examen. Los pacientes que no lleguen temprano tendrán que reprogramar la cita para una fecha y hora posteriores.
